Off-Campus College Activity
When students enrolled in a class are required to participate in or attend an off-campus college activity under control of Palm Beach State College, each student must complete the following forms:
- Release, Hold Harmless and Indemnification Agreement Release, Hold Harmless and Indemnification Agreement Student Participant
- Emergency Information & Medical Treatment Form Emergency Information and Medical Treatment Form - Student Activities
